As part of Connections for Health's response to COVID-19, we created a user-friendly Food Resource Guide with information on 73 food providers across our region. Using data provided by UNH Cooperative Extension in its online NH Food Access Map, the index of the Food Resource Guide includes for each provider the days and hours of operation, food offered, availability of delivery and pick-up, location, and contact information.

 

For each of the three region's sub-divisions (Strafford County, seacoast Rockingham County, and central Rockingham County) there is also a calendar that makes it easy to see where and when people can find resources each day of the week.  Also in the guide is a link to an interactive Google Map of all organizations listed so directions to any are easily located.

Connections for Health (CfH, formerly Region Six IDN) is committed to transforming the delivery of integrated care (clinical and non-clinical) to Medicaid members throughout our region. Our investments aim to build capacity to improve and sustain population health and wellness.

Connections for Health initiates scholarship opportunities and responds to requests to provide financial support for conferences and training identified by partner organizations that meet these shared objectives:

  • Building Agency & Network Capacity to Deliver High Quality Integrated Care

  • Transform Systems to Sustain Integrated Care Practices 

  • Enhance Readiness for Alternative Payment Models

 

The Scholarship Request Form is intended to balance a low-burden process with assisting our Operations Team to demonstrate that funding authorization decisions meet basic standards of fairness and equity whilst meeting the above stated shared objectives.

  • SCOPE of Pain: Safe/Competent Opiod Prescribing Education

FREE online training with CMEs for providers and nurses.

SCOPE of Pain is a series of continuing medical education/continuing nursing education activities designed to help you safely and effectively manage patients with acute and/or chronic pain, when appropriate, with opioid analgesics. The program consists of a free 2-module, case-based online activity and live conferences held around the US.

  • Find a Mental Health First Aid Course

            Mental Health First Aid is a course that teaches you how to help someone                  who may be experiencing a mental health or substance use challenge. The                training helps you identify, understand and respond to signs of addictions                 and mental illnesses.
