Connections for Health facilitates a Community Care Team representing partnered agencies throughout southeastern New Hampshire.

The Community Care Team network welcomes and includes direct care coordination representatives from over 60 multi-sector partners, including 4 hospitals, 3 Federally Qualified Health Centers, 2 Community Mental Health Centers, and numerous Substance Use Treatment providers, Homeless Shelters, Mental Health Clubhouses, Recovery Organizations, Public Housing Agencies, Welfare Departments, and many more.

Typical CCT Attendees include: CFH Care Coordination Team; MCO Care Managers; Case Managers; Care Coordinators; Social Workers; Nurse Care Managers; Community Health Workers; Homeless Outreach Workers; Housing Support Services, and more.
